Aging Referral Agencies
Carle Community Wellness & Faith Community Health Services
Mission: To improve the health of communities by bridging healthcare and faith-based settings, focusing on prevention, wellness, and holistic care
What they offer:
Free or low-cost health screenings (blood pressure, cholesterol, etc.).
Chronic disease education and lifestyle coaching.
Advance-care planning and connections to Carle medical resources.
Works with churches and other faith groups to host health events and health ministries.
Champaign County DHS Family Community Resource Center
Mission: To provide equitable access to human services, supports, programs, and resources to Illinois residents in need
What they offer:
Local office for public benefits: SNAP (food assistance), Medicaid, and cash assistance (Temporary Assistance for Needy Families)
Helps with child care subsidy, emergency food, and housing referrals
Guides residents to mental health, substance use, and other supportive services.

Mission: Supporting people across the generations by providing quality human services

Circle of Friends Adult Day Center

A warm, home-like setting with peer companionship, recreational and meaningful social activities.
Flexible attendance: full-day or partial-day options, multiple days per week.
Caregiver support: providing relief (respite) by caring for older adults during day hours.
Amber Glen Alzheimer’s Special Care Center
Mission: They aim to ensure people living with memory loss continue to lead meaningful lives. They begin by learning about each resident’s passions, family, hobbies, achievements, etc., and they design care that engages them emotionally, physically, mentally, socially, and spiritually

Specialized memory care for residents with Alzheimer’s disease or other dementias.
Personalized programming based on each individual's history, preferences, and interests.
Support not just for safety, comfort, and supervision, but also focused on emotional well-being, relationships, and quality of life.

Montessori Memory Care: a special memory care neighborhood that uses Montessori-based approaches, emphasizing resident-led activities, preserving independence, and purpose.

Safety, monitoring, support: 24/7 staff, secure building, wearable alert systems, resident monitoring, medication oversight, high-quality nutrition (scratch kitchen, special diets).
Legal and Financial Help
Champaign County Regional Planning Commission
Mission: RPC describes this service as helping household maintain or gain access to transportation so that lack of mobility does not become a barrier to employment, health care, or housing stability. It’s about promoting self-sufficiency and preventing secondary issues like homelessness due to lack of transit
What they offer:
Assistance with vehicle repairs (minor repairs) when a car is needed for work, health, or other essential uses.
Annual public bus passes for people meeting eligibility (low income, receiving certain benefits, etc.).
Helps preserve transportation independence so individuals aren’t forced to give up jobs or forego medical appointments because they can't get around.
